By accessing or using our website (www.proactivehealthierlife.com) and related services, you agree to comply with and be bound by these Terms of Service (“Terms”). If you do not agree, please do not use our site or services.
2. Our Services
ProActiveHealthLLC provides preventive health screenings designed to identify potential health risks early. These screenings are non-diagnostic and not a substitute for medical advice, diagnosis, or treatment. You should always consult with your healthcare provider regarding any medical concerns or test results.
3. Eligibility
You must be at least 18 years old to use our services. By booking a screening, you confirm that the information you provide is true, accurate, and complete.
4. Appointments, Cancellations & Refunds
We value your time and aim to provide a flexible and transparent scheduling experience.
Cancellations Made More Than 48 Hours in Advance
If you are unable to attend your scheduled screening and cancel more than 48 hours before your appointment, we will issue you a ProActive Health Gift Card for the full amount paid.
This gift card can be used by you — or transferred to a friend or family member — for any future screening service.
Cancellations or Reschedules Within 48 Hours
If you cancel or reschedule less than 48 hours before your scheduled appointment, a $50 cancellation fee will be deducted from the gift card amount issued to you.
This fee helps cover operational costs related to your reserved appointment time.
No-Show Policy
If you fail to attend your scheduled appointment and do not contact us in advance, no refund or gift card will be issued.
However, we encourage you to contact us — we’re always happy to help you reschedule and stay proactive with your health.
Refund Requests
As screenings are reserved and prepared in advance, we do not issue cash refunds.
All eligible refunds are issued as ProActive Health Gift Cards, which never expire and may be used toward any future preventive screening.
5. Payment and Pricing
Payment is required at the time of booking.
We accept all major credit cards and secure payment methods provided through Shopify.
All prices are listed in U.S. dollars.
We reserve the right to adjust pricing or promotions at any time without prior notice.
6. No Medical Diagnosis or Treatment
Our screenings are designed to provide early insights into potential health risks but do not replace a full medical evaluation.
We encourage you to share your results with your physician for proper interpretation and follow-up care.
ProActiveHealthLLC and its representatives are not liable for any medical decisions or actions taken based on screening results or information provided.
7. Privacy
Your privacy is important to us.

8. Limitation of Liability
To the fullest extent permitted by law, ProActiveHealthLLC, its employees, and contractors shall not be liable for any direct, indirect, incidental, or consequential damages arising from:
